Customers are required to obtain a Return Material Authorization (RMA) Number when returning product for any reason. You may request an RMA number using the following form. Please submit via fax to 817.548.1358 or by phone at 817.274.7553 between 8 A.M. and 5 P.M. Central Standard Time. Items can only be returned by the company that originally purchased the product.
